We are not in-network with: United Healthcare, Cigna, Aetna, Medicaid of WA or OR. We do not take Personal Injury Protection (PIP) or Workers' Compensation claims.

 

If we are considered an "Out of Network" provider, we will bill you directly for services. After your statement is paid in full, we can provide you with a superbill to submit to your insurance for potential reimbursement.
