What to know before you book an appointment:


You may need a referral.

• If you have Medicare or a Medicare Advantage Plan you need a referral.*

• We only require a referral if your insurance requires it.

• Learn more and access our referral form on our referral page.

* Please be aware that if you have a Medicare Advantage plan this policy replaces your Medicare benefits.

Know your insurance benefits.

 Please visit our insurance page to view the current list of insurance companies we are in-network with. 

• We also suggest you call the number on the back of your insurance card to check your benefits and any requirements. 

If you are ready to book an appointment please give us a call (please have your insurance cards available!). You may also fill out the form below.

So we may check your benefits and get you scheduled, after submitting your request, please send us a photo of the front and back of your insurance card, and the front of your photo ID, via our HIPPA secure upload link found on our document upload page.
