Prior to your first appointment, we will send easy-to-use online intake forms. This gives our providers a better understanding about your medical health history and journey with your symptoms.

If you are a new patient to Vestibular Therapy Specialists, plan to upload:

• Your Photo ID (License - front only or Passport)

• The FRONT & BACK of your insurance card(s)

• Any other documents you feel would be helpful for our providers in regards to your previous or current treatment.
